HEALTH ON THE STREET: NEIGHBORS HELPING NEIGHBORS WITH THEIR HEALTH

 

Health on the Streets is an integral component of the Future Builders Community health Initiative.  It was modeled after similar successful programs throughout the nation, which emanated out of a request from community members who said they wanted someone who "looks like me and talks like me" to improve the quality of health and human services in the community.

 

Health on the Streets utilizes Community Health Advocates (CHA) and local community volunteers who work to:  increase community knowledge of health care resources; improve community access to health care services; increase community participation in health education programs; maintain contact with persons at risk due to health-related problems; identify community needs and obstacles to quality health care.
